What Makes TRT Blood Test Results Shift?

Time of day is the most studied variable. In men aged 30-40, testosterone levels measured at 4 p.m. averaged 20-25% lower than the 8 a.m. value (Brambilla et al., 2009). Why Do Guidelines Require Two Morning Draws?

Do Other TRT Monitoring Labs Also Fluctuate?

Yes, though in different patterns than testosterone.

Hematocrit rises on testosterone. Both the Endocrine Society and the AUA recommend stopping testosterone and evaluating the client when hematocrit exceeds 54%. But hematocrit also shifts with hydration, altitude, smoking, and sleep apnea. A single elevated reading should be confirmed with a repeat draw before a lasting dose change. A complete blood count is a standard part of TRT monitoring for this reason.

Estradiol depends on the same assay-method concerns as testosterone. Direct immunoassays at male-range estradiol concentrations are less reliable than LC-MS testing (Bhasin et al., 2018). Switching from a standard estradiol assay to a sensitive estradiol test can change the reported number without any real change in the man's estradiol. For men on TRT, the Endocrine Society recommends measuring estradiol when symptoms of excess aromatization appear, not as a routine serial test for every man.

PSA has its own biological variation. Clinical guidelines treat a confirmed rising trend, not a one-time blip, as the signal for urology referral. A PSA test before starting testosterone is an AUA clinical principle for men over 40.

SHBG is relatively stable through the day (Brambilla et al., 2009) but shifts over months with thyroid status, body weight, insulin resistance, and liver function. A change in SHBG can move total testosterone without a matching change in free testosterone. When SHBG is suspected, the Endocrine Society recommends free testosterone by equilibrium dialysis or a validated calculation, not an analog immunoassay.

Does the Laboratory Itself Change the Number?

It can. Two labs running different assay platforms on the same blood sample will not always return the same value. The CDC HoSt program exists for this reason.
