A men's hormone panel typically starts with testosterone, but thyroid and adrenal markers may warrant inclusion in that panel when symptoms overlap or a pituitary problem is possible. The American Thyroid Association and the American Association of Clinical Endocrinologists (ATA/AACE) identify serum TSH as the single best screening test for primary thyroid dysfunction in most outpatient settings (Garber et al., 2012). For adrenal concerns, the Endocrine Society recommends morning serum cortisol paired with plasma ACTH as the first step, not a saliva kit (Bornstein et al., 2016).

A men's hormone panel is a set of blood tests that measures hormones across the reproductive, thyroid, and adrenal axes. Clinicians use the results, along with symptoms and medical history, to evaluate whether a hormonal deficit, excess, or secondary cause explains a client's presentation. Panels sold direct to consumers vary in scope, so understanding which tests clinical guidelines actually recommend, and in what order, prevents both overtesting and missed conditions.

What Does a Men's Hormone Panel Include?

Most men's hormone panels cover testosterone (total and free), estradiol, SHBG, LH, FSH, and prolactin. That list reflects markers the AUA and the Endocrine Society reference when evaluating androgen deficiency (Mulhall et al., 2018; Bhasin et al., 2018).

Broader panels add thyroid markers (TSH, free T4), adrenal markers (cortisol, DHEA-S), metabolic markers (fasting insulin, HbA1c), and inflammatory markers (hs-CRP). The question isn't whether these exist on a menu. The question is which ones a specific man's symptoms and history call for.

Two groups of men are most likely to benefit from thyroid and adrenal additions. The first is any man whose low testosterone follows a secondary pattern (low T with low or normal LH), because pituitary disease can suppress the thyroid and adrenal axes at the same time (Fleseriu et al., 2016). The second is any man with fatigue, weight changes, or mood shifts that don't resolve after testosterone has been evaluated. Which Thyroid Tests Should Men Get First?

TSH is the starting point. The ATA/AACE 2012 guideline calls it the best single screening test for primary thyroid disease in outpatient adults (Garber et al., 2012). One number, drawn from a standard blood sample, tells a clinician whether the pituitary is asking the thyroid to work harder (high TSH) or to slow down (low TSH).

Free T4 is the next step. If TSH comes back high, free T4 on the same sample shows how much active hormone the thyroid is actually producing. If TSH is low, free T4 and free T3 together help distinguish hyperthyroidism from other causes.

TPO antibodies are measured once, not repeatedly, when TSH is above the reference range. A positive result identifies autoimmune thyroiditis (Hashimoto's) as the likely cause and can change the monitoring plan. NICE NG145 (updated 2023) follows the same sequence and explicitly advises against repeating TPO antibodies.

Free T3 alone is not recommended by current guidelines for diagnosing primary hypothyroidism or adjusting levothyroxine. The Endocrine Society and AACE Choosing Wisely campaign advise against ordering T3 to assess levothyroxine dose. Despite this, many direct-to-consumer panels include free T3 and reverse T3 as standard items, which inflates cost without changing diagnosis in most primary thyroid cases.

One important exception: when pituitary disease is suspected (secondary hypogonadism with low or normal LH, visual field changes, or other hormone axis losses), TSH alone is not enough. A normal TSH can coexist with a low free T4 in central hypothyroidism. Fleseriu et al. (2016) recommend measuring TSH and free T4 together in that setting and treating based on free T4, not TSH. How Are Adrenal Hormones Tested in Men?

The Endocrine Society's 2016 guideline on primary adrenal insufficiency (PAI) recommends testing when specific clinical signs are present: unexplained volume depletion, hypotension, hyponatremia, hyperkalemia, hyperpigmentation, or unexplained hypoglycemia (Bornstein et al., 2016). The first test is a morning serum cortisol drawn around 8 AM, paired with plasma ACTH.

If morning cortisol falls below about 5 mcg/dL with ACTH more than twice the upper reference limit, the picture strongly suggests PAI. If morning cortisol lands in an intermediate range (roughly 5-15 mcg/dL and the clinical picture isn't clear), the standard confirmatory test is a 250 mcg cosyntropin stimulation test. Peak cortisol below 18 mcg/dL at 30 or 60 minutes (assay-dependent) indicates adrenal insufficiency.

Late-night salivary cortisol has a different purpose entirely. It is an Endocrine Society-endorsed first-line test for suspected Cushing syndrome, which may present with progressive central weight gain, wide purple striae, proximal muscle weakness, or resistant hypertension (Nieman et al., 2008). It is not a tool for nonspecific fatigue.

DHEA-S can support an adrenal insufficiency differential when it is low for age, because the adrenal cortex produces most circulating DHEA-S. A low DHEA-S in the context of low cortisol and high ACTH adds weight to a PAI diagnosis. On its own, DHEA-S declines with age and is not an indication for supplementation in men with nonspecific symptoms.

Is Adrenal Fatigue a Real Diagnosis?

No. The Endocrine Society states there is no scientific proof that adrenal fatigue is a true medical condition, and no test can detect it (Endocrine Society, 2022). A 2016 systematic review screened 3,470 articles, included 58 studies, and found conflicting results across salivary cortisol methods with no substantiation for the diagnosis (Cadegiani and Kater, 2016).

The concern goes beyond semantics. Labeling nonspecific fatigue as "adrenal fatigue" can delay identification of conditions that have established treatments: hypothyroidism, sleep apnea, depression, iron-deficiency anemia, and actual adrenal insufficiency (which has a prevalence of roughly 100-220 per million in Western populations, per data cited in the Bornstein 2016 guideline). Months spent on unproven "adrenal support" supplements are months without a morning cortisol and ACTH draw.

Four-point salivary cortisol profiles sold as "adrenal stress indexes" are a separate product from the guideline-endorsed late-night salivary cortisol used in Cushing evaluation. Some marketed panels blur the two. One is a validated diagnostic tool for hypercortisolism. The other lacks scientific support as a clinical test.

Do Testosterone Lab Panels Need Thyroid and Adrenal Markers?

The AUA 2018 guideline does not list TSH or cortisol among its required adjunct tests for testosterone deficiency evaluation. What the AUA requires is two morning total testosterone values, LH, and prolactin if LH is low or normal (Mulhall et al., 2018).

That absence doesn't mean thyroid and adrenal labs are irrelevant. It means they're indicated when the clinical picture calls for them, not as a default panel add-on.

Hypothyroidism is one reason to check. The Endocrine Society lists hypothyroidism among conditions that decrease SHBG, which can affect free testosterone measurement (Bhasin et al., 2018). In a small study by Donnelly and White (2000), free testosterone roughly doubled after levothyroxine treatment in 10 men with primary hypothyroidism. Kumar et al. (2020) found testosterone below 230 ng/dL in 29 of 40 hypothyroid men, with levels recovering in all but one after the thyroid was treated. Both are small studies. Individual responses vary. But they suggest a question providers may consider before initiating testosterone: is the thyroid contributing to this result?

When Should Men Get Hormone Panel Testing?

Testosterone levels peak between about 7 and 10 AM. Morning blood draws are required for accurate testosterone assessment, and the AUA requires two separate morning values before making a diagnosis. Thyroid and adrenal markers are less time-sensitive, but morning cortisol must be drawn in that early window to be clinically useful.

Fasting for 8-12 hours improves the accuracy of insulin and glucose measurements that often travel with hormone panels. Clients should also ask their provider about biotin. The FDA warned in November 2017 that biotin can interfere with hormone immunoassays, and a JAMA study by Li et al. (2017) showed that 10 mg per day for 7 days interfered with 9 of 23 biotinylated assays, including a 94% TSH drop on one platform. Hair-skin-nail supplements commonly contain 5-10 mg of biotin. Stopping biotin at least 48-72 hours before blood work can help reduce the risk of false results that mimic Graves disease.

How to Read Hormone Panel Results

Laboratory reference ranges represent the middle 95% of tested populations. Falling "within range" does not always mean a result is clinically insignificant, and falling outside it does not always mean disease.

For thyroid labs, context matters. A TSH of 3.5 mIU/L in a man with no symptoms and stable labs is different from that same TSH in a man with fatigue, constipation, and a rising trend. The ATA/AACE guideline recommends treatment when TSH is above 10 mIU/L and individualizes for TSH between the upper reference limit and 10 (Garber et al., 2012).

For adrenal labs, a single afternoon cortisol of 8 mcg/dL is not meaningful. Cortisol follows a diurnal curve, peaking in the morning and dropping through the day. Basing treatment decisions on a random or afternoon value can create iatrogenic problems, including secondary adrenal insufficiency from unnecessary glucocorticoid use.
