TRT blood work is one of the most important safety measures for men on testosterone replacement therapy. Without scheduled lab draws, hematocrit can climb undetected, estradiol can drift out of range, and a provider has no data to adjust the dose. The American Urological Association (AUA) and the Endocrine Society both recommend testing at baseline, at 3 to 6 months after starting treatment, and every 6 to 12 months once levels stabilize (Mulhall et al., 2018; Bhasin et al., 2018). Nearly half of men treated with testosterone never have their levels rechecked after starting therapy, according to AUA data. That gap is where preventable side effects happen.

Testosterone replacement treats a diagnosed deficiency, not a lifestyle preference. The FDA updated TRT labeling in February 2025 to include a blood-pressure warning following these data, while removing the prior cardiovascular boxed warning (FDA, 2025).

Those secondary findings are the reason hematocrit, blood pressure, and symptom review stay on the calendar even after the boxed warning came off.

How Often Should TRT Blood Work Be Done?

The schedule isn't one fixed interval. It changes based on the treatment phase and the testosterone formulation used.

What Labs Are Needed Before Starting TRT?

Confirming testosterone deficiency typically requires two early-morning total testosterone measurements below 300 ng/dL on separate days, plus symptoms consistent with hypogonadism. That threshold comes from the AUA (Mulhall et al., 2018). About 30% of men whose first value falls in the low range will test normal on repeat, which is why a single draw doesn't confirm the diagnosis (Endocrine Society, 2018).

Baseline labs also include hematocrit, hemoglobin, a lipid panel, PSA for men over 40, and LH/FSH to separate primary from secondary hypogonadism. SHBG levels and free testosterone are added when total testosterone sits near the cut-off or when obesity, thyroid disease, or other SHBG-altering conditions are present.

How Often During the First 3 to 6 Months?

The first on-treatment testosterone draw isn't the next morning. Timing depends on the formulation:

  • Gels, patches, and intranasal products: 2 to 4 weeks after starting

  • Short-acting injections (cypionate, enanthate): after 3 to 4 injection cycles

  • Long-acting testosterone undecanoate: between the first two 10-week injections

  • Pellets: 2 to 4 weeks after implant, then again at 10 to 12 weeks (Marek Diagnostics does not offer pellets)


These windows come from AUA expert-opinion guidance (Table 7 of the unabridged guideline). Drawing before steady state can lead to dose changes that don't reflect the actual regimen.

At 3 to 6 months, the Endocrine Society recommends a full safety check: testosterone (timed to formulation), hematocrit, symptom review, and PSA when age-appropriate. This is the visit that catches early erythrocytosis. In one observational series, injectable short-acting esters raised hematocrit above 50% in 66.7% of men, with a mean time to that rise of 10.5 months (Pastuszak et al., 2015). Individual rises happen sooner.

What About Long-Term TRT Blood Work?

Once levels stabilize and symptoms are well controlled, AUA recommends testosterone testing every 6 to 12 months (Mulhall et al., 2018). The Endocrine Society sets annual labs as the floor after the 3 to 6 month check (Bhasin et al., 2018). After any dose change, the clock resets: the new formulation or dose needs another 6 to 8 weeks to reach steady state before labs mean anything.

Yearly is the minimum for a stable client. It is not the maximum if hematocrit is drifting or symptoms shift.

What Labs Should Be Checked on TRT?

Total and Free Testosterone

Total testosterone is the primary monitoring analyte for both the AUA and the Endocrine Society. Free testosterone (by equilibrium dialysis or a validated calculation) is added when SHBG is altered or when total testosterone is near the boundary but symptoms persist.

Timing of the draw relative to the dosing schedule matters. For cypionate or enanthate on a weekly cycle, the Endocrine Society recommends aiming for 400 to 700 ng/dL measured about one week after the injection (mid-interval). A Monday-morning trough and a Thursday mid-interval on the same dose can differ by hundreds of ng/dL. If the draw point isn't specified, the number is uninterpretable. Same lab, same assay, same time relative to dose, same time of day.

Hematocrit and Hemoglobin

Testosterone stimulates red blood cell production. A complete blood count tracks hematocrit and hemoglobin. When hematocrit rises above 54%, both the AUA and the Endocrine Society recommend stopping therapy until it falls, evaluating for hypoxia and sleep apnea, and restarting at a lower dose (Bhasin et al., 2018; Mulhall et al., 2018). Some providers use 50% or 52% as an earlier trigger; that is practice variation, not the guideline stop rule.

The erythrocytosis rate varies by formulation. A network meta-analysis of randomized trials found mean hematocrit increases of +4.0% for intramuscular enanthate/cypionate, +3.0% for gel, +1.6% for long-acting undecanoate, and +1.4% for patch versus placebo (J Urol. DOI: 10.1097/JU.0000000000002188). This is why providers may recommend more frequent early monitoring for injectable users than for gel users.

Estradiol

Estradiol testing is not required at every visit by U.S. guidelines. The AUA recommends sensitive estradiol at baseline when gynecomastia is present and leaves other use optional. The Endocrine Society's core on-treatment triad is testosterone, hematocrit, and prostate-risk evaluation.

Estradiol becomes a targeted test when symptoms appear: gynecomastia, marked fluid retention, or mood instability. Suppressing estradiol without a clinical reason may worsen bone density and lipid profiles. The test is useful when guided by symptoms, not as a score to drive down at every visit.

PSA, Lipids, and Other Supporting Labs

PSA testing is done before treatment and at 3 to 6 months in men over 40, then per standard age-based screening. The AUA calls for urology referral if PSA rises more than 1.4 ng/dL in any 12-month period on treatment, or if confirmed PSA exceeds 4.0 ng/dL (Mulhall et al., 2018).

Lipid panels and liver enzymes provide cardiovascular and metabolic context. Blood-pressure monitoring now carries an explicit FDA warning behind it. These markers round out a monitoring plan that goes beyond testosterone numbers alone.

What Happens When TRT Blood Work Is Skipped?

The most harmful monitoring error is skipping hematocrit during year one. Injectable esters raise hematocrit more and sooner than other formulations. The mean time to hematocrit above 50% on short-acting injections was 10.5 months in one series (Pastuszak et al., 2015). That is exactly when a "things are going well, I'll skip the 6-month draw" decision hits.


Without labs, testosterone levels can drift outside the therapeutic range in either direction. Running too high creates its own risks: acne, fluid retention, mood instability, and elevated estradiol from excess aromatization. Running too low means the treatment isn't working and the underlying deficiency remains uncorrected.


The AUA also has a stop rule for the other direction. If total testosterone is in range at 3 to 6 months but symptoms haven't improved, the provider and client should discuss whether to continue. A therapeutic level without corresponding symptom improvement may warrant a supervised trial off therapy (Mulhall et al., 2018).

Why Does Draw Timing Affect TRT Lab Results?

This is the detail that most monitoring guides skip. There are two clocks running at the same time on TRT.

The first clock is pharmacokinetic: has the formulation reached steady state, and was the draw timed to peak, mid-interval, or trough? A weekly cypionate injection typically produces a peak within 24 to 48 hours that can read well above 700 ng/dL, while the day-7 trough on the same dose may sit near 400 ng/dL. Both numbers are from the same person on the same protocol. The Endocrine Society specifies mid-interval (about day 7 of a weekly cycle) and recommends 400 to 700 ng/dL for that window. For gels, the draw should happen 2 to 8 hours after application.

The second clock is safety: hematocrit, PSA, blood pressure, and symptoms. These don't depend on where the draw falls in the injection cycle, but they do require a consistent calendar.

Mixing these two clocks can lead to premature dose changes and missed side effects. A provider who understands blood work preparation and formulation-specific draw windows will order labs that actually reflect the regimen.

The assay also matters. Reference ranges vary across labs. The Endocrine Society asks for CDC-certified or externally quality-controlled assays because methods differ. Switching labs mid-treatment can produce apparent "crashes" or "spikes" that are just assay variation, not a real change in hormone status.

Fertility and TRT Blood Work: A Separate Conversation

Exogenous testosterone suppresses LH and FSH, which commonly reduces or stops sperm production. The AUA states directly: do not prescribe exogenous testosterone to men currently trying to conceive (Statement 23, Strong Recommendation). Alternatives like SERMs, hCG, or aromatase inhibitors may be used when fertility preservation is the goal (Mulhall et al., 2018).

LH and FSH are mainly diagnostic and fertility-planning tests, not quarterly TRT scorecards. Suppressed LH/FSH on exogenous testosterone is expected. Recovery of spermatogenesis after stopping is variable and not guaranteed on any specific timeline.

Any man considering testosterone replacement should have a fertility conversation with his provider before starting. Skipping that conversation may lead to delayed conception and the possible need for assisted reproduction.
