Why Does Preparation Affect Results?

In men aged 30-40, the afternoon value is typically 20-25% lower than the 8 a.m. value (Brambilla et al., 2009). The gap narrows with age (about 10% at age 70), but the morning-draw recommendation applies regardless.
The AUA specifies "early morning" draws. The Endocrine Society says "morning, fasting." Both point to the same 7-10 a.m. window. Clients who work nights or rotate shifts should discuss timing with their provider, because sleep itself (not the clock) drives part of the testosterone rhythm. Axelsson et al. showed that testosterone rose during sleep whether that sleep was at night or displaced to daytime (Axelsson et al., 2005).

Can Medications or Supplements Skew Results?
Several categories of medications suppress testosterone at the testicular or hypothalamic level. Chronic opioids, glucocorticoids, and some anticonvulsant drugs are the most common. The practical rule is don't schedule the draw immediately after exhaustive training the morning of the appointment. A routine session the day before is unlikely to move the needle by the 18-28% that day-to-day biological variability already introduces (Brambilla et al., 2007).
Sleep matters more. Even one week of restricted sleep (5 hours per night) lowered daytime testosterone by 10-15% in healthy young men (Leproult & Van Cauter, 2011). The Endocrine Society also instructs against diagnosing hypogonadism during acute or subacute illness, recent surgery, or other acute stress. Anyone acutely ill should reschedule the draw.

Diagnostic Draws vs. On-Treatment Monitoring
Diagnostic blood draws and on-treatment monitoring draws serve different purposes and follow different preparation rules. Mixing up the two is the most common source of unusable lab results.
A diagnostic draw answers the question: is testosterone low? The protocol is two fasting morning draws on separate days, same lab and assay if possible, with LH and FSH included to distinguish primary from secondary hypogonadism. The AUA calls a total testosterone below 300 ng/dL a reasonable cutoff (Mulhall et al., 2018). The Endocrine Society's harmonized lower limit is 264 ng/dL (Bhasin et al., 2018). The same blood sample can be classified differently depending on which reference the provider uses. Preparation can't fix a mismatched cutoff, but it can prevent a false low from a fed afternoon draw.
An on-treatment monitoring draw answers a different question: is the current protocol producing safe and appropriate levels? The timing depends entirely on the formulation (see the table below), and the fasting rule may reverse. Hematocrit and PSA are part of the same visit, not optional add-ons. The Endocrine Society recommends checking hematocrit at baseline, at 3-6 months, and annually, and recommends reassessing the protocol (including possible dose reduction or suspension) if hematocrit exceeds 54% (Bhasin et al., 2018).
On every requisition, record the hours since the last dose, the formulation, and whether the draw was fasting or fed. That context turns a number on a lab report into something a provider can act on.

What to Expect on the Day of a Hormone Blood Test
Clients should arrive at the lab with a requisition, a photo ID, and a normal hydration state. Dehydration can raise hematocrit. Overhydration can lower it. No society recommendation specifies a water intake target, but the goal is a usual hydration state, not one influenced by an overnight flight or a sauna session.
The draw itself takes less than 5 minutes. A phlebotomist draws blood from a vein, usually in the arm. Minor bruising at the site is common and resolves in a day or two. If the same panel includes a lipid panel, fasting glucose, or HbA1c, the fasting morning draw covers all of them in a single visit. The ACC/AHA 2018 cholesterol guideline notes that a nonfasting lipid profile is acceptable for cardiovascular risk screening (Grundy et al., 2019), but since testosterone still requires fasting, one fasting morning visit serves both purposes.
