Most physicians don't refuse testosterone testing to be dismissive. They follow clinical guidelines that recommend against screening men who don't show symptoms of deficiency. That doesn't mean testing is unavailable. Testosterone deficiency, also called hypogonadism, is a clinical and biochemical condition in which the testes produce insufficient testosterone. Diagnosis requires both persistently low serum testosterone on at least 2 early-morning blood draws and the presence of compatible signs or symptoms (Mulhall et al., 2018). Neither a single lab value nor symptoms alone qualify. Why do some doctors decline testosterone testing?
The decision often reflects clinical guideline positions rather than dismissiveness, though individual physician factors may also play a role. The Endocrine Society recommends that clinicians not screen asymptomatic men for low testosterone (Bhasin et al., 2018). The American Urological Association agrees that testing should follow symptoms or specific high-risk conditions, not general curiosity (Mulhall et al., 2018).
There are practical reasons too. Testosterone has a wide diurnal swing. A single random afternoon draw can return a low number that doesn't reflect the man's actual hormonal status. Time-limited primary care appointments often don't leave room for the full clinical evaluation that a testosterone workup demands (2 confirmed morning draws, a symptom assessment, possible LH/FSH to classify the cause). And many symptoms men attribute to "low T" (fatigue, low mood, weight gain, reduced drive) overlap with obesity, untreated sleep apnea, depression, opioid use, and uncontrolled diabetes. A physician is trained to rule those out first.
None of this means the concern is invalid. It means the path to testing runs through a specific set of criteria.
Conditions that warrant testosterone testing
The AUA endorses testing in men who present with persistent symptoms of testosterone deficiency. Those symptoms, stated as clinically associated in both the AUA and ES guidelines, include reduced libido, erectile dysfunction, fatigue, loss of muscle mass, increased body fat, depressed mood, poor concentration, and impaired memory (Bhasin et al., 2018).
The AUA also identifies conditions where testing is appropriate even without classic symptoms. These include unexplained anemia, bone-density loss, type 2 diabetes, prior chemotherapy or testicular radiation, HIV/AIDS, chronic opioid use, pituitary dysfunction, and chronic corticosteroid therapy (Mulhall et al., 2018). Type 2 diabetes has been associated with an increased prevalence of testosterone deficiency in published data, which is one reason the AUA includes it in the recommended testing category (Mulhall et al., 2018).
The takeaway is clear. Doctors don't test without symptoms because guidelines tell them not to. But guidelines also define specific situations where testing is medically indicated, and men in those categories have grounds to ask.
How is testosterone deficiency diagnosed?
A single blood test doesn't confirm deficiency. The AUA guideline calls for 2 total testosterone measurements drawn on separate mornings under fasting conditions during the early-morning window (Strong Recommendation, Grade A) (Mulhall et al., 2018). The ordering provider determines the exact timing and scheduling. This matters because testosterone peaks in the early morning and drops through the day. An afternoon sample can read meaningfully lower than the same man's early-morning value, which is why guidelines specify morning draws.
The AUA defines a threshold it considers a reasonable cutoff for low total testosterone (Moderate Recommendation, Grade B). The Endocrine Society describes the diagnostic bar as "unequivocally and consistently low" on a reliable assay, and its July 2026 statement acknowledges that different societies and labs use slightly different thresholds (Endocrine Society, 2026). The treating clinician interprets the result in context. No single number applies equally to every man.
Once a low level is confirmed, the clinician should measure LH and FSH to distinguish primary hypogonadism (a testicular problem) from secondary hypogonadism (a pituitary or hypothalamic problem). This classification changes the treatment approach and may reveal underlying conditions, including pituitary tumors, that require their own workup.

Total testosterone, free testosterone, and lab variability
Total testosterone measures both protein-bound and free hormone. Free testosterone, the small fraction not bound to sex hormone-binding globulin (SHBG) or albumin, is the biologically active form. The Endocrine Society recommends checking free testosterone by equilibrium dialysis or a validated calculated method when total testosterone is near the lower limit of normal or when SHBG is known to be altered (Bhasin et al., 2018).
This distinction matters in practice. Obesity is associated with lower SHBG, which can pull total testosterone down while free testosterone may remain adequate. Aging and liver conditions tend to raise SHBG, which can push total testosterone into a normal range while free testosterone drops. A man told his levels are "fine" on the basis of a single total testosterone may have a free testosterone problem that was never checked.
Lab variability adds another layer. A U.S. laboratory survey found that lower reference limits for total testosterone varied widely across labs, with some setting the floor far lower than others and most using manufacturer-supplied ranges rather than locally validated ones (Le et al., 2016). The same blood sample can fall inside one lab's normal range and below another's. The CDC's Hormone Standardization Program (CDC HoSt) certifies total testosterone assays against a reference standard for accuracy, but not all labs use a certified method. The Endocrine Society's 2026 statement flags nonstandardized assays and variable reference ranges as a direct cause of misclassification.
For men using individual biomarker testing or testosterone panels, knowing which assay a lab uses and whether it carries CDC HoSt certification matters for result accuracy. Asking the lab directly is reasonable.
Does social media fuel unnecessary testing demand?
A growing body of evidence says yes. In November 2025, U.K. endocrinologists reported that social media misinformation, particularly on TikTok and Instagram, was driving men to seek testosterone testing and therapy they didn't need. Professor Channa Jayasena of Imperial College London, chair of the Society for Endocrinology's andrology community, told journalists that NHS specialists across the U.K. were seeing these men every week: "They're filling our clinics. We used to see people with adrenal problems and diabetes, and it's really affecting NHS care."
A study evaluating TikTok content accuracy for men's health topics scored testosterone-related content at 2.3 out of 5 on a misinformation index, where physician-created content scored higher but represented a small fraction of total posts (Halpern et al., 2022).
The pattern isn't limited to the U.K. In the U.S., social media and direct-to-consumer marketing have reframed testosterone testing as a lifestyle gateway rather than a diagnostic step for a specific medical condition. The Endocrine Society's 2018 guideline and its 2026 statement both position testosterone replacement as treatment for documented deficiency, not a supplement for healthy aging, energy, or body composition goals (Bhasin et al., 2018).
This matters for men with genuine symptoms. Unnecessary demand strains the clinicians and systems that serve them. And it explains part of why doctors have become cautious about ordering the test on request alone.
TRT formulations: more than a shot
A common misconception is that testosterone replacement therapy means injections. It doesn't. The FDA lists multiple approved routes, and the choice depends on pharmacokinetics, side-effect profile, adherence factors, fertility plans, and the risk of skin transference to household members.

The AUA recommends commercially manufactured products over compounded testosterone when possible (Conditional, Grade C). Risks and required monitoring on TRT
Testosterone replacement therapy carries real risks that require regular monitoring. The TRAVERSE trial, the largest cardiovascular safety study of TRT to date (5,246 men, ages 45-80), found that the primary cardiac endpoint (cardiovascular death, nonfatal MI, nonfatal stroke) occurred at similar rates in the testosterone and placebo groups: 7.0% versus 7.3% (Lincoff et al., 2023). That neutral primary finding doesn't tell the full story. The testosterone group had higher rates of atrial fibrillation (3.5% vs 2.4%), acute kidney injury (2.3% vs 1.5%), and pulmonary embolism (0.9% vs 0.5%).
In February 2025, the FDA issued a class-wide labeling update for all testosterone products. The agency removed the previous cardiovascular boxed-warning language, added TRAVERSE results and a blood pressure warning based on ambulatory monitoring data, and retained the limitation-of-use statement for age-related low testosterone (FDA, 2025).
Other monitoring concerns include erythrocytosis (elevated hematocrit from increased red blood cell production), which can raise the risk of blood clots. Men with untreated sleep apnea face compounded risk because low oxygen already drives red blood cell production. The AUA recommends measuring hemoglobin and hematocrit before starting treatment and at regular intervals after (Mulhall et al., 2018).
Fertility suppression is another consideration. Exogenous testosterone commonly suppresses gonadotropin signaling along the hypothalamic-pituitary-gonadal axis, which can reduce or halt sperm production. The AUA states that men currently trying to conceive should not start exogenous testosterone (Strong Recommendation, Grade A). Recovery after discontinuation is variable in both timing and completeness, and full restoration of fertility cannot be assumed.
PSA monitoring may be appropriate based on age and risk profile. Prostate cancer rates in TRAVERSE were not meaningfully different between groups in the study window, but the Endocrine Society's 2026 statement notes that long-term prostate safety is not established and monitoring remains needed.
The AUA guideline recommends that providers discuss discontinuation if testosterone reaches the therapeutic range but symptoms don't respond within the timeframe the guideline specifies. TRT is not a permanent default. Its continuation depends on measurable clinical response, assessed by the treating provider.
How to ask a doctor for testosterone testing
A productive conversation with a provider typically starts with what has changed rather than a target number. Describing specific symptom changes (reduced libido, persistent fatigue beyond what sleep and stress explain, loss of muscle mass without changes in activity, depressed mood) gives the clinician the information the guidelines use to determine whether testing is appropriate. The provider's evaluation determines next steps.
Relevant medical history matters too. Type 2 diabetes, sleep apnea, chronic opioid use, and prior pituitary issues all fall within the AUA's recommended testing categories, even without classic symptoms. Sharing that history helps the provider make the call.
If testing is ordered, guidelines call for early-morning fasting samples drawn on separate occasions. The ordering clinician determines exact timing and scheduling. A single afternoon draw can return a misleadingly low value, which is why the protocol exists.
